MM4M offers a unique service to expectant moms. The company has expanded it services to the greater Eastern Cape region. Currently we are serving the Nelson Mandela Bay Metro which comprises Port Elizabeth, Uitenhage and Despatch.
Regions outside PE includes Grahamstown, Port Alfred, Jeffreys Bay and Humansdorp.Instead of endless frustrations and waiting in hours long queues or travelling long distances to the nearest UIF office, MM4M offers to:
* Provide you with the necessary maternity application forms, and submit the application on her behalf to the Department of Labour’s UIF offices. For moms in regions outside Port Elizabeth, the forms must be posted to MM4M: PO Box 66, Swartkops, 6209.
* Submit the continuation of payment forms (UI-4) to the Department of Labour on the mom’s behalf.
(Should the mom require MM4M to deliver & collect the UI-4 form from her gynaecologist for confirmation of birth as required by UIF, an additional fee is applicable.) This however is OPTIONAL.
